Program Requirements

Body Mass Index: Most insurers require a BMI of 40 or above, or between 35 and 40 with a weight-related health condition, such as diabetes, high blood pressure, or sleep apnea. The FDA has approved the Lap-Band for use in patients with a BMI as low as 30 with a weight-related health condition, although most insurers have not yet lowered their BMI requirements. BMI requirements and most other requirements are the same whether you are having a Lap-Band or a Sleeve Gastrectomy. In order to be eligible for a Gastric Balloon, patients must have a BMI between 30 and 40.

BMI Limits:  Men must have a BMI less than 55 and Women must have a BMI less than 60 at the time of surgery.

Age: You must be at least 18 years old and less than 65 years old at the time of surgery.

Insurance: We take most major insurance plans, including Blue Cross, Harvard Pilgrim, Tufts, Commonwealth Indemnity, Neighborhood Health Plan, BMC Health Net and certain United Healthcare plans. We do have a cash-pay option for the Lap-Band for those patients whose insurance does not cover the procedure.
